Before filling out a registration form, check you are in our catchment area.
Find out which GPs you can register with on the NHS website.
Registering with the surgery
To register with the surgery you can do any of the following:
- complete the NHS ‘Register with a GP surgery’ form online (or you can print a copy from here)
- visit reception from 10am to 6pm and collect a paper registration form
When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.
Fill out a register for online services form also, if you wish to have online access to your records.
If, when you register with us, you wish to have a ‘new patient health check’, please contact the surgery.
