Use this service to do request a non-NHS doctors letter.
You can use this service if you:
- are registered at the surgery
Private letters are not part of the practice’s NHS contract and therefore there are charges for this work, there may also be a delay in processing the request as NHS contract work will take priority. For this reason, we cannot provide a specified time in which it will be completed. The GP is within their rights to reject this private work.
Requests from patients for letters to support applications to Department of Work and Pensions (DWP) and Personal Independence Payments (PIP) in relation to benefit claims will not be undertaken. It is not a requirement of DWP or PIP that Doctors provide written evidence; it is your responsibility to complete these forms detailing how the illness or disability for which you are claiming is affecting you. DWP contact us directly if they wish us to provide any information or a report to assist with your claim.
Any requests for letters for schools will also not be undertaken.
Before you start
We’ll ask you for:
- your first and last name, date of birth, sex, postcode, email and phone number
- if applicable, the details of the person you are completing the form on behalf of
The surgery’s fee for a short letter is £20, other documents may be considerably more and advice can be sought before any work is undertaken. The charge will need to be paid upon collection of the letter.
