Child ADHD/Autism Right To Choose – Information for parents

Right to choose referrals are patient (or in this case Parent) initiated referrals, and we cannot specifically recommend providers due to you needing to make your own decision on which provider to use.

There are a lot less providers for children than adult referrals, but please feel free to have a look at the following providers we have previously referred to to see if they are a provider you would like to use:

Each service will have their own individual pathways, and some will require you to complete their company specific questionnaires for the GP to review before the referral can be made.

Once you have chosen a provider please complete any questionnaires required and return them to the surgery. A GP can then be informed and if appropriate, they can complete the referral.
