Private fees are payable for certain services, including certain types of medical examination, passport signatures, holiday cancellation forms and private insurance notes. The fees payable are detailed in our Schedule of Patient Fees.

DWP and PIP requests in relation to benefit claims
The Practice is receiving an increased number of requests from patients for letters to support applications to Department of Work and Pensions (DWP) and Personal Independence Payments (PIP) in relation to benefit claims.
It is not a requirement of DWP or PIP that your doctor provides written evidence; it is your responsibility to complete the form detailing how the illness or disability for which the claim is being made affects you.
Please do not ask for a letter to support your claim as this will not be provided by your GP, however, you can ask at reception if doctor is willing to provide a summary printout of your condition to put with your claim. This will not be dealt with urgently and any request has to be approved by doctor before the printout can be provided. Our secretarial team will call you to let you know about the outcome of your request and if a printout can be provided when and where to collect, it will not be posted.
Please do not make a doctor appointment regarding these requests as they will be dealt with using the same process as requesting via reception.
DWP and PIP have their own medical assessors who will write directly to your GP if they require further medical information.

Form SC2 can be completed by you to cover a period of absence from work; this is available from your employer. If you are sick for more than 7 days your employer may ask you for a Fit Note from your doctor, in this case you should advise the receptionist before booking an appointment as a telephone consultation may be sufficient.
If you are self-employed or unemployed and have been sick for 4 days or more you may be entitled to benefits, further information can be obtained by calling 0800 055 6688 or visiting Job Centre Plus.
