Please follow the advice below:

  1. Visit enter your destination(s) and view the advice and vaccinations recommended and complete the Travel Assessment Form below. If you do not have internet access please go to step 2.
  2. If you think you require NHS vaccines or if you are unsure what vaccines are required, please complete the Travel Assessment Form within this pack.
  3. If you think you require NHS vaccines, please book a routine travel appointment with Mandy Down or Kath Watson as any NHS vaccines can still be administered here, free of charge. It is important to make an appointment at least 6 weeks before you travel to ensure you are fully informed and immunised, as this allows time for the vaccines to work.

What happens next with your form:

It is really important that you bring your completed Travel Assessment Form with you to your travel appointment so it can be assessed by the nurse. If you complete the form online, this will be available at your appointment. They will be able to offer general travel advice and provide the NHS vaccines you require. Without your Travel Assessment Form the nurse may not be able to vaccinate and a further routine appointment may be required.

Please note – If private travel vaccines are required these may need to be given over a longer period of time so you may wish to contact a private travel service at your earliest convenience.
